I, IMA A. DONOR, now living in ___________________, Ohio, hereby declare that this is a Codicil to my Last Will and Testament, dated ________ (date of existing Will).
1. I hereby amend my Last Will and Testament to add a new ITEM ___ to read in its entirety as follows:
“ITEM ___. I give the sum of ___________________________ Dollars ($ _______ ) to the Columbus Bar Foundation, or its lawful successor, for use as the governing body of the Foundation shall determine from time to time.”
2. I hereby amend my Last Will and Testament to renumber the succeeding Items in order.
3. In all other respects, I hereby ratify and affirm my Last Will and Testament as amended by this Codicil.
