Court Adopts New Probate Forms

The Supreme Court has issued seven new standard probate court forms for representation of insolvency cases filed pursuant to Ohio Revised Code statute 2117.15. They were prepared by the Forms Committee of the Ohio Association of Probate Judges and recommended by the Commission on Rules of Superintendence.

The forms were published earlier this year for a 30-day public comment period. They take effect January 1, 2012.

Amendments to the Rules of Superintendence for the Courts of Ohio cover Standard Probate Forms 24.0 through 24.6. They include:

Standard Probate Form 24.0 – Representation of Insolvency
Standard Probate Form 24.1 – Judgment Entry Setting Hearing and Ordering Notice
Standard Probate Form 24.2 – Notice of Hearing on Representation of Insolvency and Schedule of Claims
Standard Probate Form 24.3 – Verification of Service: Notice of Hearing on Representation of Insolvency and Schedule of Claims
Standard Probate Form 24.4 – Insolvency Schedule of Claims
Standard Probate Form 24.5 – Continuation Insolvency Schedule of Claims
Standard Probate Form 24.6 – Judgment Entry of Insolvency
